 Swan Health Genetics is a private genetic counseling practice owned and operated by a licensed, board-certified genetic counselor. I offer services to individuals and families residing in the United States.

  • State of residence

Please note: Do not submit personal health information (PHI) through the website contact form. PHI is collected only through secure, HIPAA-compliant intake forms after you book an appointment.

 Information submitted via the contact form is used solely to respond to your inquiry. It is not sold or shared with third parties for marketing purposes.

If you become a client, additional personal and health information will be collected through secure intake processes and stored using HIPAA-compliant platforms.

4. How Information is Stored and Protected
 Contact form submissions are delivered via email to my Google Workspace account, which is configured to meet HIPAA compliance standards. While these messages may temporarily contain identifiable information, I do not store PHI long-term in email.

All client health information is securely stored in Carepatron and Google Workspace, both of which are HIPAA-compliant platforms. Reasonable administrative, technical, and physical safeguards are in place to protect your information.

5. Third-Party Services
 I use the following third-party services:

  • Carepatron: For secure intake, session documentation, and storage of PHI

  • Google Workspace: For secure communication and recordkeeping

  • Squarespace: For hosting and managing my website

These third parties have their own privacy and security practices and are selected to align with HIPAA compliance requirements.

6. Cookies and Website Analytics
 At this time, I do not use cookies or analytics tracking on my website.

7. Your Rights and Choices
 You may request access to or correction of your personal information at any time. If you have submitted a contact form and wish to withdraw your inquiry or have your information deleted, please email me directly.
